The MH Program Specialist (Complex Cases) positions are with the Westmoreland County BHDS office located in downtown Greensburg behind the Courthouse. The County MH Program Specialist I Complex Case (dual diagnoses) will be a liaison between the mental health and intellectual disability systems both in house and with the necessary MH/IDA stakeholders in the community. The Program Specialist will act as the point person for dually diagnosed cases and will be responsible for the development of a behavioral health care management meeting for each individual. They will bridge the gap between the individual, their medical services, health systems, and community resources with care coordination tasks. They will assist with intervention strategies and support in crisis situations, treatment services, and discharge/re-integration to the community. The Program Specialist will support and ensure engagement with physical health service needs, as well as offer the individual a medium for open communication between service systems and providers.This position will also require specific reporting and data collection responsibilities that will be used in identifying best practices and targeting performance improvement efforts. Will be responsible for assessing and coordinating the care needs for consumers identified as Seriously Mental Ill (SMI) and dually diagnosed with an intellectual disability or Autism (IDA) diagnosis. The Program Specialist will provide assessment, education, planning, care coordination, identification of resources, and service facilitation from a person-centered approach with the goal of increasing the quality of life and prioritize their needs and goals.
The Behavioral Health Program Specialist (PS) for Complex Cases is a position that will be responsible for assessing and coordinating the clinical care needs for adult consumers identified in two or more of the following groups:1) being discharged from psychiatric inpatient or state hospitals;2) admissions to Long Term Structured Residence or any other long term care facility3) at risk for multiple inpatient readmissions;4) Seriously Mentally Ill (SMI) consumers experiencing reoccurring acute episodes; or5) a member of a priority or special needs population (i.e. Intellectually Disabled, Aging, Forensic, etc.) who present with complex behavioral health, developmental services, medical and/or other life care needs.Additionally, the PS will participate in the development and evaluation of clinical guidelines or standards of care for the previously identified populations of patients. The PS collects, trends, and analyzes aggregate outcomes data to improve outcomes for complex consumers. The PS will identify best practices and target performance improvement efforts.
