We at PAHrtners Deaf Services are happy to announce our Blended Case Management (BCM) program has expanded to include school-aged children and adolescents from ages 4 – 21. These services are in addition to our already established adult case management program.
We serve students who have a mental health diagnosis with serious and persistent behavioral health challenges, who also are Deaf, Hard of Hearing, DeafBlind, or are hearing children of Deaf parents.
Services include:
- Accessing and understanding community resources
- Advocacy
- Crisis intervention
- Enhancement of support network
- Linking individuals with appropriate service providers
- Monitoring service delivery
- Post-graduation planning
- Treatment plan development and maintenance
Our case managers work to breakdown interagency barriers to seamlessly blend educational, psychological, medical and social services.
We are able to offer BCM services to students in the following counties:
- Armstrong
- Butler
- Indiana
- Lawrence
- Washington
- Westmoreland
Allegheny coming soon! We can also contract with other counties as needed.
Services are provided by individuals who are fluent in sign language and well-versed in Deaf culture. Case managers will travel to the student’s home, school, medical appointments or other locations as required.
