The link below is for a survey which was developed by the Strategic Plan Workgroup of Governor Wolf’s Opioid Command Center (OCC). The survey is designed to gauge how interested stakeholders receive information about statewide opioid initiatives. This survey will assist the OCC and the Strategic Plan Committee in developing future direction and the structure of opioid-related initiatives and meetings.
