These services are based upon the completion of an assessment and are coordinated by an Area Agency on Aging Care Manager.
Program Description
Adult Day Care Programming for rehabilitation and socialization of older adults
Aging Waiver Program Provides home and community based services to financially eligible persons age 60 or older as an alternative to nursing facility placement
Assessments Long term care options
Care Management Coordination of home and community based services through a care plan developed to meet individual needs
Caregiver Relief Hands-on care while relieving regular caregiver
Home Delivered Meals / Meals on Wheels Accommodates need for daily nutritious meal
Home Health Non-Medicare services with a doctor's order required.
Nursing Home Transition Moving from a Nursing Home to a community setting
Personal Assistance Services Hands-on care with daily living tasks to frail older persons with chronic health problems
Personal Care Assistance with personal hygiene.
Placement (Domiciliary Care) The placement and monitoring of individuals 18 years or older who require supervision in AAA certified Domiciliary Care Homes
