Are there other Benefits to the Dom Care Program? |
Yes. Consumers residing in Dom Care homes are provided on-going care management services through a Westmoreland County Area on Aging Care Manager. Care management services provide a way of securing needed services for individuals currently residing in Dom Care homes. Through social, physical and psychological assessments, the Care Manager is able to determine the needs of the individual and provide follow-up to meet those needs. The Aging Care Manager will advocate for essential services needed for either the consumer or home provider to continue the community living arrangement. |
