Nursing Home Transition is a program in Pennsylvania which assists people to move from a Nursing Home to a community setting with the assistance of home and community based services. The process entails counseling consumers in Nursing Facilities regarding long term care options, providing information on a full range of community services and coordinating transition back to the community.
Long Term Counseling is available to all interested consumers. Counselors will visit nursing facility residents and provide information and guidance to consumers and their families.
Although 24 hr care is not available, a Personal Response System can be provided and used to call for assistance in serious emergencies.

• Home-delivered meals
• Personal Care Services
• Personal Response Systems
• One time payments for Security Deposit, Utilities, Moving Expenses, Household Goods and furniture
• Medical Services Equipment
• Nursing Services
• Physical, Speech and Occupational Therapy
• Respite for caregivers
• Medical Supplies
