OPTIONS Care Management

The OPTIONS Care Management program provides to qualifying seniors in-home services primarily focusing on personal care needs and ancillary home support assistance to meet individual needs.

A care manager works with the consumer/ family to determine needs and how these needs are best met. The care manager accomplishes this determination through a comprehensive assessment. Once an assessment is completed and the consumer is found eligible, the care manager, with consumer input, develops a care plan addressing need. Thereafter, the care manager will remain in contact with the consumer to ensure the services are provided as indicated in the care plan and continue to meet the consumer’s needs.

The services of the OPTIONS Care Management program range from the primary service of personal care (bathing, dressing, grooming, toileting etc.) to laundry, shopping, adult day care programs and nutritional need screening for home delivered meals. Services are designed to be specific to individual needs and preferences, and aimed to provide care choices to people in need while supporting that person’s health, safety, wellbeing and ability to remain in their own home.
